Whanau & Community
All of our health services are delivered through our Whanau Ora Integrated model of care.
Whānau Ora is a new way of working with whanau. It focuses on whanau as a whole rather than individual whanau members. It allows whanau to develop their own aspirational plans to ensure whanau reach their maximum potential. Whanau are then able to drive their own plans and initiatives, overcoming barriers quickly and with ease.
Whanau can also be guided through this process by a “Whanau Ora Navigator”. A Whanau Ora Navigator is able to walk alongside whanau and coordinate whanau needs to ensure whanau tell their story once, their experience is good and they get the results that ensure they are better off.
Our Whānau Ora kaiarahi (navigator) will work with you to help identify your needs and help you to develop support plans towards achieving your dreams and aspirations. Whānau Ora works collaboratively to improve experiences when accessing services and walks alongside whānau as they make informed decisions about their health and wellbeing.
Whānau Ora takes a holistic approach to wellbeing, which means taking into account the overall wellbeing of the whānau in the following areas:
- Mauriora - cultural connectedness and support systems
- Toiora - Physical Health, Mental Health & addictions, Family Violence
- Waiora – Housing & Environment
- Te Mana Whakahaere – Income Management & Insurance
- Te Oranga - Employment, Education, Resourcing & Community Engagement
